To request information on how to obtain your records please fill out the below form and our office will contact you. Please note that there maybe a small fee associated with your record request depending on the size of your chart.

**Please note we can not forward medical records to another facility if you do not have an appointment. Please ask your provider to fax for medical records once you have an appointment: 603-444-0274 OR call if you have an questions at 603-444-0272 ext 1.
